This type of transformation is used in
- GDAL: gdalwarp
  http://www.remotesensing.org/gdal/

- GRASS 5.0.x: i.rectify
  http://grass.itc.it

Here it is used for raster data.

There is also
- GRASS 5.0.x/5.1.0: v.transform
  which does transformations on vector data.

As all is Free Software, you are able to study the (documented) algorithms.
